Clinical Observation of 78 Cases of Blunt Abdominal Trauma

Abstract
Blunt abdominal trauma is often a challenging problem to the surgeons, because of difficulty in establishment of early accurate diagnosis and sometimes requirement of urgent operation, hence resulting in relatively high morbidity and mortality. Recently blunt abdominal trauma is more frequent in proportion to the increase of automobile accident.
This report consists of 78 cases of blunt abdominal trauma treated at the General Surgery Service of Capital Army Hospital from April 1963 to August 1968, with brief review of literatures.
1) Of 78 cases, 38 cases(48.5%) were by automobile accident, 26(33.2%) by blow and kick, 11(14.2%) by fall and 3(3.8%) by unknown etiology.
2) The most common clinical manifestations were abdominal pain, tenderness, rebound tenderness and rigidity.
3) Diagnosis was made by history and clinical manifestation. The X-ray study and paracentesis were helpful.
4) The injured organs were 32 cases (29.6%) of small bowel, 16 of spleen, 10 of liver, 8 of pancreas, 7 of kideney, and the others.
5) In the most cases, operative procedures were performed as below; primary closure for perforated bowels, removal of ruptured spleen and kidney ans hemostasis or partial resection with adequate drainage for injured liver.
6) Whole blood transfusion was required in 38 cases. Massive transfusion was required in a half of 16 cases of splenic rupture.
7) The most common complications were wound infection, pulmonary complication, and fistula.
8) The mortality rate was 7.6%(6 cases) and 2 cases died of hypovolemic shock.
